Under the supervision of the Program Manager, the System of Care (SOC) Coordinator will advocate for specific student needs/family needs at the school site and function as a coordinating liaison between ACP, Reid Park CMS staff and SOC participants, and homeless families.   


  • Interact with school principals, social workers, families, volunteers, teachers, donors, and other SOC agencies to assist in meeting the critical and immediate needs of homeless families in order to normalize the student's school day experience
  • Identify child and/or family needs based on presented agency goals and resources with a focus on erasing the impact of homelessness on attendance and behavior. These needs may include medical, mental health, and basic needs.
  • Provide supportive services to ensure children's homeless crisis does not impede their ability to be present in school, prepared and successful during the school day
  • Identify and implement interventions to reduce the frequency of children's behavioral and attendance issues during the school day
  • Manage caseload as directed by the CMS McKinney Vento list and System of Care referrals
  • Offer opportunities for and monitor parent involvement
  • Participate in agency meetings, SOC case consultations, and school team meetings
  • Establish and maintain positive relationships with all CMS and SOC staff
  • Collaborate with CMS Social Worker to provide community-based referrals as needed
  • Maintain accurate and timely data and record keeping as defined by the agency regarding all services provided including ACP volunteer services
  • Facilitate distribution of in-kind donations to students as determined by observation and/or referral
  • Coordinate with ACP Volunteer Department and maintain strong volunteer services and relationships during the school day
  • Assist in providing homeless awareness services to CMS school sites
  • Assist with fundraising activities and community outreach events and meetings that benefit ACP
  • Develop and monitor case plans and referrals for children and/ or families on the CMS McKinney Vento list
  • Travel to home visits as needed. This may extend into after-school hours, up to 6:00 p.m.
  • Use the Generalist Social Work model to engage, assess, plan, intervene, evaluate, terminate, and follow up with the children and parents on the caseload


  • Master of Social Work degree
  • Two or more years' experience working with at-risk youth
  • Strong written and verbal communications skills
  • Strong knowledge and understanding of generalist Social Work practice
  • Able to work well as a team member in school, agency, and community settings
  • Ability to handle multiple assignments and meet deadlines
  • Reliable transportation (This position requires less than 50% travel)
  • Consent to a pre-employment background check and drug screen


  • Two years of social work experience with children and families in a community or school based organization
  • Experience working with the homeless population
  • Understanding of the systemic challenges of poverty
