The Care Ring Nurse Case Manager (NCM) comprehensively plans for case management of targeted patient populations. The NCM assesses needs then plans, coordinates and evaluates services of patients with the goal of equipping and empowering individuals and their families to easily access resources and adopt healthy lifestyles that will increase their ability to remain healthy at home or in the least restrictive environment.

Basic Job Functions

  • Responsible for population management, health education, clinical triage, and complex case management, including chronic disease care management, complex medication management, and critical diagnosis care coordination
  • Works collaboratively and maintains active communication with the patient's multidisciplinary care team including providers, pharmacists, social workers, behavioral health specialists, and nurses to achieve timely, appropriate patient management.
  • Interacts with patients, professionals, and the community to achieve continuity of care, coordination of services and to document plans of care.
  • Carries a case load of clients and appropriately utilizes various computer databases to keep all information accurate and complete.
  • Conducts or participates in comprehensive "all-system" needs assessment for identified patients.


  • Follows protocols of care related to Well-Woman Health Maintenance, Care Gap Closure, and Chronic Disease Management.
  • Supports policies, procedures, guidelines and standards of care for Care Ring.
  • Functions as liaison for patient/family in navigating the continuum of care. Serves as patient advocate.
  • As requested/required, participates in community outreach activities.
  • Continually improves knowledge through individual reading and research, attendance at professional organization meetings, in-service and community workshops.


  • Knowledgeable of appropriate care -related services to match identified needs, disease management for health maintenance, and appropriate clinical goal expectations/outcomes for identified populations.
  • Maintains knowledge of and establishes relationships with community resources for referrals and care partnerships and utilizes those to meet patient needs.
  • Conducts evaluative/consultative visits in numerous settings.
  • Provides support services and/or appropriate referrals for client and family as needed.
  • Develops and maintains accurate case records of each referred customer/patient. Documents fully and accurately.
  • Knowledgeable of and appropriately utilizes EHR for documentation and case review
  • Proactively identifies and resolves delays and obstacles impeding clinical progress.
  • Utilizes advanced conflict resolution of issues.
  • Maintains patient confidentiality at all times.
  • Focuses health care activities on the achievement of optimal outcomes, resource utilization, clinical expertise, and improvement strategies.
  • Appropriately recognizes and provides for cultural, language, and age-specific differences of the patient/family.
  • Performs related duties as assigned or required.


  • The Covid-19 vaccination is required as a condition of employment.
  • BSN required. Licensure in the State of North Carolina.
  • Two years' experience in health care.
  • BLS Certification.
  • Valid driver's license and insured automobile.
  • Bilingual in Spanish preferred.
  • Clinical competence in disease management and case management principles.
  • Ability to work independently and as a member of the care team.
  • Excellent written and verbal communication.
  • Strong computer skill and proficiency in Microsoft applications.
  • Must be able to work a flexible schedule including some evenings and weekends.
  • Two years' recent experience in Population Health/Case Management including discharge planning in acute care, home care, LTC care, Physician Office or Managed Care Company.
  • Excellent interpersonal communication and negotiation skills, problem-solving skills, strong organizational and time management skills.
  • Knowledgeable of services available in the community.
  • Knowledge of barriers to health care for the low-income, uninsured population.
