Community Health Worker

The Care Ring CHW is community-based position that provides case management, coordination of care and health education to Healthy Start, Inc. program participants. This position serves as the first line of care in the Healthy Start, Inc. model. As a trusted member of and/or has a unique understanding of the community being served, the Community Health Worker has a passion for maternal and child health and advances family systems and life course approaches to support improved birth outcomes, and family health and well-being. The CHW will serve as a liaison between health/social services and Healthy Start participants in the community. The CHW will facilitate access to services and work to provide high quality and culturally competent service delivery in their scope. A Healthy Start CHW also supports participant and community capacity, using their experience and knowledge, by increasing perinatal health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.

Basic Job Functions

  • Identify and enroll eligible participants based on Healthy Start program qualifying criteria.
  • Provides home and community-based care coordination and linkages to community resources and to program participants and their families to ensure access to appropriate Healthy Start supports and services.
  • Provide evidence-based and evidence-informed education including, but not limited to: breastfeeding, immunization, safe sleep, family planning, smoking cessation, nutrition, mental health, physical health, stress, domestic violence.
  • Screen for depression using the Edinburg (women) and the PhQ9 (men) and refer to treatment as necessary.
  • Assist in the development and management of the participant's plan of care/goal planning'including birth and reproductive life plans.
  • Demonstrates open and respectful communication in all interactions with program participants and their families to positively represent the Healthy Start Federal Project in the community.
  • Maintains accurate records of program participants and submits information in a timely manner to meet grant productivity requirements.
  • Ensures that program activities and documentation conform to federal laws and organizational guidelines and policies to protect participant's personal information.
  • Complete all training and ongoing professional development related to the Healthy Start model, public health and health disparities. Attend meetings, events and conferences as necessary.
  • Supports policies, procedures, guidelines and standards of Care Ring and the Healthy Start model.
  • As requested/required, participates in community outreach activities.
  • As requested/ required, promotes public awareness of Care Ring and Healthy Start model.
  • Develops and maintains relationships with community agencies and healthcare providers.

Skills/Knowledge

  • Maintains knowledge of the Healthy Start program.
  • Possess knowledge of prevention strategies, needs and risk assessments, emergency response and crisis management skills for family systems.
  • Reports suspected cases of child and/or adult abuse/neglect/exploitation. Serves as liaison between provider, hospital, patient/families and Department of Social Services during evaluation and investigation.
  • Thoroughly and accurately documents all patient interactions, social work assessments, interventions and planning; ensures records facilitate evaluation and monitoring of applicable interventions.
  • Utilizes conflict resolution skills as necessary to ensure timely resolution of issues. Can make decisions independently and as part of a team using sound analytical and problem solving skills.
  • Ability to facilitate group sessions.
  • Demonstrates basic knowledge of medical terminology in all interactions.
  • Utilizes effective communication skills to collaborate with the multidisciplinary healthcare team.
  • Maintains patient confidentiality at all times.
  • Evaluates client progress toward stated goals and documents outcomes.
  • Appropriately recognizes and provides for cultural, language and age-specific differences of the patient/ family.
  • Performs related duties as assigned or required.

Minimum Requirements

  • Applicants must reside in Priority Health Areas, which includes the following zip codes: 28205, 28206, 28208, 28212, 28216, and 28217.
  • A high school diploma or GED and three or more years of experience in a social service/health field. Bachelors Preferred.
  • Combinations of education and experience will be considered. Experience working with home visiting and/or populations experiencing health disparities.
  • Valid driver's license and insured automobile.
  • Excellent written and verbal communication skills.
  • Basic computer skills.
  • Must be able to work a flexible schedule including some evenings and weekends.
  • Knowledgeable of services available in the community preferred.
  • Knowledge of barriers to health care for the low-income uninsured population preferred.
