Social Worker

The Care Ring Social Worker assesses needs, plans, coordinates and evaluates services of Physicians Reach Out patients with the goal of equipping and empowering individual enrollees and their families to function as independently as possible within their community. The Social Worker provides educational support, crisis intervention and information/referral services to patients, families, and staff. Additionally, the position will build relationships with community agencies and primary care providers, and assist with patient outcome monitoring.

Basic Job Functions

  • Interviews, assesses needs, and refers patient/families to resources for social, emotional, or financial assistance when appropriate and serves as a patient advocate to help in the decision-making process.
  • Maintains knowledge of community resources for referrals and care partnerships and utilizes these to meet patient needs.
  • Carries a caseload of clients and appropriately utilizes various computer databases to keep all information accurate and complete.


  • Supports policies, procedures, guidelines and standards of Care Ring.
  • As requested/required, participates in community outreach activities.
  • As requested/ required, promotes public awareness of Care Ring.
  • Develops and maintains relationships with community agencies and healthcare providers.


  • Completes all required education and training.
  • Maintains knowledge of the PRO program.
  • Interviews patient/family to complete a comprehensive psychosocial evaluation as needed.
  • Provides formal/ informal education to staff and physicians/ providers to help them better understand the emotional, social, and psychological components of the patient's illness and its effect on the family unit.
  • Performs in-home patient assessments as needed to address care needs and improve patient outcomes.
  • Reports suspected cases of child and/or adult abuse/neglect/exploitation. Serves as liaison between provider, hospital, patient/families and Department of Social Services during evaluation and investigation.
  • Thoroughly and accurately documents all patient interactions, social work assessments, interventions and planning; ensures records facilitate evaluation and monitoring of social work interventions.
  • Utilizes conflict resolution skills as necessary to ensure timely resolution of issues. Can make decisions independently and as part of a team.
  • Demonstrates basic knowledge of medical terminology in all interactions.
  • Utilizes effective communication skills to collaborate with the multidisciplinary healthcare team.
  • Maintains patient confidentiality at all times.
  • Evaluates client progress toward stated goals and documents outcomes.
  • Appropriately recognizes and provides for cultural, language and age-specific differences of the patient/ family.
  • Performs related duties as assigned or required.

Minimum Requirements

  • Bachelor's degree in Social Work or related field.
  • Prefer at least 2 years of experience with indigent population in some previous capacity.
  • Valid driver's license and insured automobile.
  • Bilingual in Spanish.
  • Excellent written and verbal communication skills.
  • Basic computer skills.
  • Must be able to work a flexible schedule including some evenings and weekends.


  • Master's degree in Social Work and/or Counseling
  • Two year's recent experience in hospital or healthcare setting, including opioid abuse.
  • Strong organizational, analytical, and time management skills.
  • Strong computer skills.
  • Knowledgeable of services available in the community.
  • Knowledge of barriers to health care for the low-income uninsured population.
